Campbellford Memorial Hospital – Expression of Interest – Patient Family Care Advisory Council

Campbellford Memorial Hospital (CMH) is looking for volunteers to be part of the Patient Family Advisory Council (PFAC). The group advises on matters involving patients and
families served by the hospital.

PFAC identifies and supports opportunities to improve the hospital experience and exchanges and generates ideas to enhance patient safety and quality.

The group meets once a month for 1.5 hours. The meetings are supported by Microsoft Teams and can be accessed from your home. All meeting materials are e-mailed in advance.

Take this opportunity to have an active voice in your community.

Those interested will be asked to complete an application form and must meet the following criteria:

• In the last 3 years you’ve used CMH services either directly or for a family member

• You’re comfortable sharing experiences and offering ideas that can advance patient safety and quality of care

• You’re willing to have a criminal and vulnerable sector check and to sign a confidentiality agreement
