SIU Investigating Collision on Highway 401 in Whitby that Resulted in Fatalities
Update: SIU Releases Further Details in Relation to Fatal Whitby Multi-Vehicle Collision

Seven investigators, two forensic investigators and one collision reconstructionist continue to investigate the circumstances surrounding a multi-vehicle collision that occurred in Whitby on April 29, 2024. As a result of the collision, four people died, including an infant, and two people were seriously injured.

As of today:

Subject officials: Two subject officials were designated. At this time, neither has submitted to an interview or provided their duty notes, as is their legal right.

Witness officials: 19 were designated. Interviews are ongoing.

Civilian witnesses: So far, 36 have been identified and interviewed.

Two individuals suffered serious injuries and were transported to hospital: The 27-year-old female from the Nissan Sentra was released from hospital, while the 38-year-old occupant of the cargo van remains in hospital.

Post-mortems for the four deceased individuals took place on May 1, 2024.

At this time, the SIU is in possession of more than 100 videos, obtained from: in-car camera footage, drone footage, body worn camera footage, Ministry of Transportation footage and civilian recordings.
